Please be advised that your Certification of Identification Form (Form 186) received on April 12, 2013 was rejected on May 7, 2013 because the title of the Medical School Official that certified your Form186, their title did not match the authorized signature list previously received from your medical school.



In addition, ECFMG® has received a copy of a medical diploma from the Ondokuz Mayis Universitesi Tip Fakultesi in TURKEY. However, because ECFMG does not have a sample copy of the final diploma awarded by your medical school in our reference files, we are unable to determine if the document you submitted is the official final diploma issued by your university.



Consequently, we ask that you contact your medical school to provide a sample copy of the final medical school diploma to ECFMG. The sample copy must accompany a letter signed by a Medical School Official (Dean, Vice Dean, or Registrar) on the school’s letterhead indicating that the attached sample copy is the official final medical school diploma issued by your university and that this document would allow the holder to seek medical licensure in Turkey. We also ask that the letter states the first year this diploma design was awarded.



ECFMG must receive this letter by mail to confirm that it was posted from the medical school. We cannot accept this letter by fax. This letter can be mailed to my attention at the following address:
