Dear Dr. özdemir,
Here is some information on what I can offer in a fellowship. I have been affiliated with a University in the past but am now in private practice which allows me to devote more time to voice disorders - laryngology is all that I do. Because I have a private practice, if you qualify to practice in the US, you would end up with a full Oregon license, rather than a training license. We may have already spoken about this when I was in Mumbai.

I would like to characterize my practice, so that you can be sure you would get out of a stay here what you are looking for, especially since this is a private practice fellowship rather than a university-based Fellowship.
I have a fairly low volume practice. It is devoted exclusively to disorders of larynx and while that is mostly voice, it includes breathing and swallowing problems as well. Almost all my patients are referred from other specialists or self-referred from the internet, so they have usually seen at least two doctors previously, often more. I see about 25 new patients per month and have an average of 80 return patient visits per month. I work Monday, Tuesday, Thursday (all day) and Friday morning in the office. Wednesday (all day) and Friday afternoon are my days off. Surgeries are typically in the mornings but the day of the week is quite variable.
I schedule one hour per new patient. I describe my exam in detail on the voicedoctor.net web site and it includes a substantial portion of my time listening to the patient's voice to determine what is not working. It is helpful for the fellow to be able to match the pitch of a persons voice as I do not use electronic measuring equipment to assess the pitch of the voice. A background in singing is certainly not required but is a great help in understanding some of the concepts behind my style of exam and diagnosis.
The bulk of the procedures that I perform are done in the office and include Botox injections, steroid injections, collagen injections, Laser (KTP) tuning and biopsies. Major surgical procedures in the hospital include microlaryngoscopy, pitch altering surgery (feminization laryngoplasty), reinnervation and laryngeal medializations - all described on my web site. I schedule them during my clinic days and do an average of 10 hospital cases per month. I travel quite a bit to teach, on average about one week per month, where I will be out of town or out of country.
Most of my fellows have not had a US license so that the Fellowship then is strictly observational. That is, you will be present in the office for all of my outpatient visits as well as observing in the operating room of three hospitals for surgeries. Typically fellows who stay for three months end up writing a Clinical research paper. For anyone who stays at least three months, I reimburse up to $650 per month of living expenses.
