The Cleveland Clinic Psychiatry Consult Service Orientation

GENERAL DUTIES & EXPECTATIONS:
1. You will be assigned to 1 of 3 CL teams. Each team consists of a staff psychiatrist, +/- medical students, +/- medicine/neuro rotators, +/- PA’s.
2. Rounds begin at 9am. You are expected to see your patients, write your notes and be ready for rounds by 9am daily. This will also make the rest of your day go more smoothly. Staffing rounds will also be done in the afternoon as necessary.
3. You may get up to 3 new consults per day (8:00am-4:29:59pm).
4. You will be assigned a specific week to carry the CIT pager at the beginning of the service. Complex CITs which need to be followed may count as your consult, so review it with your staff and Deb/Miles if this is the case.
5. At the conclusion of each day, update patients on your team.
6. Other expectations as follows:
Mondays Tuesdays Wednesdays Thursdays Fridays Weekends
Every Monday at 12pm, there is a mandatory group case discussion in P4 conference room —interesting cases from the prior week are discussed.
Every Tuesday at 12pm, there is a mandatory CL journal club. You will be assigned 1 journal club per CL module. Wednesday afternoons are protected time for your med check clinic and outpatient patient issues. You are not expected to see patients during didactics (1:30-3:30), but you are expected to be available for consults and rounds immediately afterwards.
There are no medical students on Fridays, so you will be the primary provider for all the patients on your team’s list. Great time for 1:1 staff interaction. You are required to round 1-2 Saturdays per module. This will be assigned by Deb/Miles at the beginning of the module.


RESIDENT TIPS:

#1) Take ownership!
You are not just “consultants” but you deliver active management on many of these patients, and thus your input is highly valued by all consulting service. Have an understanding why you are being consulted, what is your role, and which patients need or do not need to be followed.. It will be helpful each day, to engage in a discussion with your staff regarding which patients need to be seen and managed daily, can be signed off, or followed “at a distance”. This discussion is not only educational but can also assist with focused management and optimal patient care. Prioritizing patients at the beginning of the day will help maximize your knowledge of the service if you need to see an urgent consult during the morning.

#2) Medical Students
There will typically be 1-2 med students per team. We expect you to serve as a “senior” mentor to them. Although you do not directly supervise their individual consults, please make yourself available to them for questions during the day. They are inexperienced and need guidance. Any breaks during rounds (while the staff is writing notes/making calls/etc) is a great time for informal, “curbside” teaching. Please check in with them at the end of each day to make sure their patient information is updated on the team list.

#3) Keep everyone informed
The service will do their best to limit any pages sent during your clinic/didactic times. Reminding Deb or Miles about commitments early in the day will help minimize disruptions. Report any service related problems to Dr Pozuelo or the chiefs as they arise. Due to the team nature of the service, vacations are discouraged during CL months, so plan accordingly.


** CL is a busy and unpredictable hospital service. Remember that CL is a team structure. Everyone is doing the best they can and we are all here to help each other.
